Collaborative Care Program offered by JSSA
Fewer than half of parents of children with intellectual and developmental disabilities make long – term plans about who will take over their child’s care if the parent or other relative providing care dies or becomes incapacitated, a study suggests.
JSSA created the Collaborative Care Program, an effective, affordable option to address this growing concern of individuals and their parents. The Collaborative Care Program focuses on helping individuals and families develop a strengths-based, team-based approach to establishing and maintaining a coordinated plan of care which ensures that all needs, from routine to crisis, are identified and managed. The model shifts the overall care approach from a person (parent)-dependent to a collaborative care team approach, using a facilitated care coordination model.
